If the breast is sagging, an implant beneath the breast tissue will sag, too. An implant behind the pectoral muscle will look terribly unnatural; the implant would be positioned high on the chest wall, with sagging breast tissue and the nipple hanging below. Your best option is a thorough consultation and examination by a board-certified cosmetic surgeon who is experienced in both breast augmentation and breast lift.
